We are seeking a Health Coach that will be a dedicated patient support person within an interdisciplinary clinical team, focusing on empowering patients with complex medical needs to actively manage their health. You will help patients gain the knowledge, skills, tools and confidence to become active participants in their care so that they can reach their self-identified goals. This may be accomplished by providing education, motivation, and guidance on lifestyle changes, medication adherence, and overall care coordination, often through regular check-ins and personalized action plans, to improve outcomes and quality of life.
What You Will Do
Serve on interdisciplinary care team as patient health coach, in partnership with a clinical RN and Pharmacist
Actively engage in patient rounds
Experience with chronic condition coaching, specifically diabetes, heart disease, and COPD.
Act as facilitator of change for assigned patients
Empower patients to set their own health and wellness goals (or support them as they strive to stick to a treatment plan from a healthcare practitioner) and offer support as they make progress
Support patients as they learn to recognize their own values, internal strengths and insights, and mobilize those assets to help them reach their goals
Offer support to develop self-management strategies to help patients make sustainable changes
Help patients understand their “why,” or their motivation for change
Empower patients to see themselves as experts on their own health and circumstances
Provide support and accountability for goal setting that is quantifiable.
Using broad knowledge of health and wellness to help clients navigate through their health concerns
Documentation of services provided in accordance with program and compliance requirements
Serve as a liaison and partner to patient caregivers.
Collaborate with healthcare providers as needed
Provide remote telephonic coaching services at a frequency based on risk, need, and program specific workflows

What will make you successful here
Bachelor’s degree in healthcare or related field preferred.
Minimum of 3 years’ experience in chronic condition clinical coaching (Frailty, Diabetes, CHF, COPD, etc.) or other relevant healthcare coaching experience
National Board for Health and Wellness Coaching (NBHWC) Certified or willing to be certified within 3 months of employment.
A passion for health and wellness
Excellent listening, written, and verbal communication skills
Empathy and emotional intelligence
Ability to build strong, trusting relationships with patients
Deep understanding of positive psychology, behavior change, and motivation
Mindfulness skills, coaching presence, problem-solving skills
Demonstrate a positive attitude and respectful, professional customer service
Acknowledge patient’s rights on confidentiality issues and follow HIPAA guidelines and regulations
Comfort in interacting as part of interdisciplinary team, confidence in speaking with physicians and medical staff regarding patient care needs.
Comfortable with digital technology (including tools like Point of Care Tools, MS Office, Google, various EMRs, etc.) and able to troubleshoot technology issues
Ability to critically think, solve problems, and bring professionalism to all situations
Able to contribute to quality improvement and process improvement initiatives
Ability to be flexible and embrace change while remaining highly organized, and detail oriented
