This often requires this individual to give and receive feedback frequently and communicate challenges or opportunities effectively.
What is the purpose of this role?
As a key member of the market leadership team, the Advanced Practice Provider (APP) Pod Leader is responsible for clinical leadership and provider team management in the market. This role combines direct patient care with people management and quality oversight responsibilities. The APP Pod Leader builds and motivates a high-performing team of advanced practice providers, ensuring Nest's ambitious goals for population health, quality, patient experience, and value-based performance are met.
The APP Pod Leader will coach and oversee performance of advanced practice providers in the market, working closely with the Market or Regional Medical Director and the VP/General Manager. This person will supervise, coach, and develop APPs delivering in-home and virtual care to pediatric and adult patients. In addition to their leadership duties, the APP Pod Leader will provide direct care via telehealth. They will foster a collaborative, accountable, and compassionate culture across the market's interdisciplinary team and serve as a key voice in advancing Nest's family- centered model of care and driving improved out comes for Nest patient families and payer partners.
What will you do?
Serve as a clinical leader and people manager a group of advanced practice providers, overseeing performance, engagement, and clinical quality.
Work with the Medical Director to ensure clinical accuracy, identify and support clinical training.
Collaborate with the market VP/General Manager and Health Services Director as part of the leadership triad and other APP Pod Leaders on market workflows, quality improvement initiatives, and to drive outcomes
Train new APPs, validating required skills and facility with Nest tools and workflows
Conduct clinical supervision and oversight, including ride-alongs, chart reviews, and feedback sessions.
Provide coaching and development support for APPs, fostering clinical excellence and a strong team culture.
Support quality improvement initiatives and ensure adherence to clinical guidelines and value-based care metrics.
Participate in interdisciplinary team meetings to ensure coordinates, high-quality care across disciplines.
Provide a small volume of direct patient care (virtual and in-home) to pediatric and adult patients in addition to supporting unexpected or prolonged APP absences when necessary.
Build and maintain collaborative relationships with community partners, payers, and referring providers.
Identify opportunities to improve clinical workflows, team tools, and care delivery processes.
Use Nest Health's electronic health record and care coordination tools for documentation, tracking, and communication.
What do you bring to the Nest?
Active and unrestricted Nurse Practitioner or Physician Assistant License in Arizona with national board certification (FNP, PNP, or similar)
Active DEA registration or eligible to apply
Minimum of 3 years of clinical experience (5+ preferred), ideally in family medicine, primary care, or home-based care
At least 1 year of formal or informal leadership experience (e.g., lead APP, precepting, mentoring, team supervision)
Passion for mission-driven work and improving healthcare for underserved communities, especially Medicaid families
Experience in a value-based or population health model is a plus
Ability to manage both clinical and administrative duties effectively
Comfortable working in a fast-paced, evolving startup environment with autonomy and adaptability
Reliable transportation and willingness to complete in-home visits
Commitment to promoting preventative care, including vaccines
What is required (travel & physical requirements)?
Primarily in-person role to provide team supervision and patient care
Occasional virtual care delivery
Occasional travel to HQ or other markets for training or collaboration
Must be able to lift up to 25 pounds
