The Role:

We are seeking a licensed Nurse Practitioner to join our growing clinical team and help redefine obesity and cardiometabolic care through FlyteHealth's integrated, outcomes-driven model.

This role combines direct patient care with clinical operations support in a technology-enabled, team-based environment. The Nurse Practitioner serves as a key clinical resource, managing patient cases, medication-related workflows, clinical escalations, asynchronous care, and care coordination to ensure timely, high-quality outcomes.

Working within a multidisciplinary care model, Nurse Practitioners collaborate closely with physicians, nurses, dietitians, diabetes educators, health coaches, medical assistants, and operational teams to deliver coordinated, evidence-based care.

This is a fully remote position.

Principal Duties and Responsibilities:

Clinical Operations & Care Team Support

  • Manage patient cases, follow-up care, and routine clinical workflows.
  • Review, triage, and resolve patient cases through Athena and approved communication platforms.
  • Provide clinical guidance to Medical Assistants and Clinical Support staff.
  • Escalate complex or high-risk cases to physicians as appropriate.
  • Support workflow improvement, protocol development, quality initiatives, and team training.

Medication Management & Clinical Case Resolution

  • Manage medication refills, titrations, and treatment optimization for obesity and related chronic conditions.
  • Review laboratory results and clinical data to support treatment decisions.
  • Assist with prior authorizations, appeals, and medical necessity documentation.
  • Address medication questions, side effects, adherence barriers, and treatment concerns.
  • Collaborate with providers on complex medication management decisions.

Asynchronous & Virtual Care Management

  • Conduct asynchronous E-Visits and Priority Visits in accordance with organizational protocols and state regulations.
  • Manage secure patient communications and follow-up activities.
  • Support continuity of care through outreach, care coordination, and patient engagement initiatives.

Direct Patient Care

  • Conduct video-based visits and deliver evidence-based, individualized care.
  • Develop and manage treatment plans that may include:
  • Lifestyle modification
  • Nutrition counseling
  • Behavioral interventions
  • Pharmacotherapy
  • Chronic disease management
  • Prescribe medications within scope of practice and educate patients on treatment expectations.
  • Monitor progress, adjust treatment plans, and coordinate referrals as needed.

Population Health & Quality Initiatives

  • Support population health programs focused on clinical outcomes and care-gap closure.
  • Participate in outreach related to medication adherence, laboratory compliance, preventive care, and chronic disease monitoring.
  • Contribute to quality improvement initiatives and organizational performance metrics.
  • Maintain compliance with regulatory and organizational requirements.

About You:

  • Passionate about obesity medicine, weight management, nutrition, and chronic disease care.
  • Strong written and verbal communication skills.
  • Comfortable utilizing EMR/EHR systems and virtual care technologies; Athena experience preferred.
  • Knowledge of evidence-based obesity and cardiometabolic care.
  • Demonstrates cultural sensitivity, compassion, and strong patient engagement skills.
  • Able to manage competing priorities in a fast-paced virtual environment.
  • Familiar with healthcare regulations, including HIPAA requirements.

Required Qualifications:

  • Master's Degree or Doctor of Nursing Practice (DNP) from an accredited Nurse Practitioner program.
  • Active RN and Nurse Practitioner licensure in good standing.
  • Board certification through ANCC or AANP.
  • Prescriptive authority and DEA registration, as required.
  • Minimum of 2 years of experience in primary care, internal medicine, obesity medicine, endocrinology, or a related specialty.
  • Experience with EMR/EHR systems; Athena experience preferred.
  • Strong telehealth and virtual care communication skills.

Preferred Qualifications:

  • Experience in obesity medicine, weight management, endocrinology, primary care, lifestyle medicine, or chronic disease management.
  • Multi-state licensure strongly preferred.
  • Licensure in Georgia, Connecticut, New York, Delaware, Maryland, and California highly preferred.
  • Experience working within multidisciplinary care teams.
  • Experience in value-based care, population health, or virtual care delivery models.
  • Knowledge of prescription prior authorization processes and requirements.
